Biomedical content management and presentation (Medical Home Portal)
The Medical Home Portal provides information to support clinicians, parents/guardians, and other professionals in improving care and outcomes for children with special health care needs and their families. The Portal is unique in addressing the needs of both families and professionals around a wide range of chronic conditions in children and in providing information about providing optimal primary care for children with those conditions, relevant local professional and community services for each of multiple states, advocating for the children and obtaining appropriate services, including through schools, and caring also for the caregivers and families.
The Portal, established in 2001, is a project of the University of Utah Department of Pediatrics, in collaboration with the Utah Department of Health (UDOH) Bureau of Children with Special Health Care Needs and Utah Family Voices. It is led by Chuck Norlin, MD.
Our efforts with the Medical Home Portal included reorganizing its architecture, internal terminologies, maintenance workflow, and contributing to the user interface redesign.
